Harvest in the Wood Food Truck Vendor Form

Join us and be a vendor at the Harvest in the Wood Festival.  Please complete this form and our coordinator, Nicole Salser, will be in touch!

A certificate of insurance is due at time of application payment. If no insurance exists--a letter releasing the City of Ravenswood must sent with payment.
Electrical Amperage *
Complete payment $400 *
Credit card payments must be made by calling 304-273-2621. Cash payments can be made at 1 Wall Street, Ravenswood, WV 26164. Check payments may be mailed to Harvest in the Wood, 1 Wall Street, Ravenswood, WV 26164. All payments must be received by July 31.
Indemnification statement *
The concessionaire agrees to indemnify, hold harmless, and defend the sponsor(s), management or agents of Harvest of the Wood festival, collectively or individually, for any loss of injury, that may arise or occur, by or to the independent contractor, his employees or agents, his property or merchandise, from any causes or negligence, natural or otherwise, during or while conducting any business or any other related activities by himself or his agents, in conjunction with this contract or any other agreement or contract, as authorized by the Harvest in the Wood festival.
Health Department Requirements *
All food vendors must comply with all local health codes and with any request from Jackson County Health Department officials. The Health Department will not license anyone who does not prepare food in an approved, inspected food service kitchen. The Health Department also has the right to deny any services at the event if you are not within their codes and Harvest in the Wood is not liable for the decision made by Health Department Inspectors. You also agree to be ready for inspection at the specified time one the day the event opens. Inspectors will not wait to approve your site and service may be denied by tardiness.
Rules and Regulations *
1. All vendors must be in their designated space and ready for the festival to begin promptly. Under NO circumstances will you be permitted to drive through the festival grounds after starting times. 2. You cannot change your assigned spot on your own. Consult with Nicole Salser (vendor coordinator) on any issues. 3. If vendor needs lighting, they must provide a extension cord (minimum length 50') 4. Vendor must provide a 50' minimum food grade water hose. 5. Festival hours are Thursday 5pm to 10pm, Friday 5pm to 12am, Saturday 11am to 12am, Sunday 12pm to 5pm.
Vendor agreement statement *
I/we have read the agreement rules and regulations and accept and agree to be bound by all terms in this form. I hereby release Harvest in the Wood Festival from all claims arising from my participation in the festival. All decisions of the Festival Coordinators are final. Violations of rules may result in eviction with no refund of fees. There are no refunds for cancellations by the vendor(s).
